About care plan verification extracted for each in-home care support office

Eligible in-home long-term care support offices
In-home long-term care support business establishments that use 70% or more of the standard amount for the divisional payment limit and that home-visit care accounts for 60% or more of the services used.
About the verification method
1. The city requests the submission of a care plan that corresponds to the in-home care support office.
2. The in-home long-term care support office examines the appropriateness of using the care plan, describes the reasons for the need for home-visit care, etc. in the care plan and submits it to the city.
3. The city confirms the contents of the care plan and, if necessary, discusses the care plan at community care meetings.
4. If it is determined in the discussion that a review is necessary, the in-home long-term care support office will review the care plan and care plans with similar or similar content.
