About downloading the transfer request form

On October 1, 2017, part of the "Implementation Standards for Transportation and Acceptance of Injured and Sick" was revised in Tokyo.
When transferring to another hospital, please fill in the necessary information on the transfer request form and submit it to the ambulance crew who arrived.
In addition, since October 1, 2019, there have been some changes in the content, so we have changed to the new format.
