I have moved here from another city. How do I take my child for an infant health check-up?

Update date: December 1, 2020

Children who are eligible for the city's health checkups for 3- to 4-month-olds, dental health checkups for 1-year-olds, health checkups for 18-month-olds, and health checkups for 3-year-olds are sent a notice of the health checkups in the month before the applicable month.
If your child has not received the card, please contact the Comprehensive Parent and Child Support Center.
