What kind of system is the high-cost long-term care combined medical expenses system?

For both medical care (medical insurance for the elderly aged 75 and over) and long-term care (long-term care insurance) paid by the household for one year (from August 1st to July 31st of the following year) payment amount), and if the total amount exceeds the standard amount, it will be paid upon application.
