Note: The power of attorney should be filled out by the person who made the power of attorney. (Download the application form to the "About power of attorney " page)
Items required for billing
- Documents verifying the identity of the applicant who came to the counter
- Documents confirming the authority of a legal representative such as a guardian of an adult (We may check the contents of the documents.)

Points to note when requesting a copy of your resident card
- Regarding requests for Japanese residents, unless there is a special request, the legal domicile, head of the family, relationship, name of the head of the household, resident card code, and individual number (my number) are omitted in principle.
- Regarding requests for foreign residents, unless there is a special request, the relationship, name of the head of the household, nationality/region, status of residence, etc., nickname history, resident card code, and personal number (My Number) are generally omitted.
- In the case of a third-party request, explanatory materials (relevant materials) are required according to the reason for the request.
- If a proxy requests a copy of the residence certificate or a certificate of details containing the residence certificate code or personal number (My Number), the document will be sent to the proxy by mail. Please note that these documents cannot be issued on the spot.

- Please fill in the necessary information on the document for which you are going to receive certification.
- If you make a mistake, please rewrite it on a new sheet without correcting it.
