If you wish to cancel the issuance application for some reason after applying for a My Number Card (Individual Number Card), the following procedures are required.
Note: In the case of a voluntary agent, the application cannot be canceled over the phone.

Please tell them that you want to cancel the application for My Number Card .
Hearings (interviews for identity verification, etc.) will be conducted.
(1) Identity verification documents A1 or B2 (see “List of identity verification documents” below)
In addition to the above, the following documents are required:
(1) Agent's identity verification documents A1 or B2 (see "List of Identity Verification Documents" below)
(2) Documents certifying authority of representation (family register, etc.)
Note: (2) is not required if:
・When parents and children living in Inagi City are in the same household on the resident card and the parent-child relationship can be confirmed by the relationship on the resident card ・When the permanent domicile is in Inagi City
In addition to the above, the following documents are required:
(1) Agent's identity verification documents A1 or B2 (see "List of identity verification documents" below)
(2) Power of attorney
