If you have applied for a My Number Card (individual number card) and wish to cancel your application for any reason, please follow the steps below.
Note: If you are a voluntary representative, you cannot cancel your application over the phone.

Please tell us that you would like to cancel your My Number Card application .
We will conduct a hearing (hearing to confirm your identity, etc.).
(1) Identity verification documents A1 point or B2 points (see "List of identity verification documents" below)
In addition to the above, the following documents are required:
(1) Agent's identity verification documents A1 or B2 (see "List of identity verification documents" below)
(2) Documents proving authority of representation (copy of family register, etc.)
Note: (2) is not required in the following cases:
・If the parent and child live in Inagi City, and the resident record indicates they belong to the same household, and the parent-child relationship can be confirmed by the relationship on the resident record. ・If the legal address is in Inagi City.
In addition to the above, the following documents are required:
(1) Agent's identity verification documents A1 or B2 (see "List of identity verification documents" below)
(2) Power of attorney
