If it is difficult to receive documents related to long-term care insurance at the insured person's address due to unavoidable circumstances and you wish to have them sent to an address other than the address listed on the insured person's resident card, please refer to the "Nursing Care Insurance Related Documents Sending Address". You can change the shipping address by submitting a "Setting Notification". If you wish to change or cancel a delivery address that has already been set, a similar notification is required.

★Identification documents: Current valid certificate issued by a government agency (My number card, driver's license, etc. Two items are required for health insurance cards, nursing care insurance cards, etc. that do not have a photo)
