About changing the destination of nursing care insurance-related documents

If it is difficult to receive documents related to long-term care insurance at the insured person's address due to unavoidable circumstances and you wish to have them sent to an address other than the address listed on the insured person's resident card, please refer to the "Nursing Care Insurance Related Documents Sending Address". You can change the shipping address by submitting a "Setting Notification". If you wish to change or cancel a delivery address that has already been set, a similar notification is required.

★Identity Verification Materials for Heirs If you are an heir other than a legal heir (spouse, child, grandchild, sibling, niece, nephew, etc.), a document proving your identity as an heir (such as a will) is required.


★Identification documents: A currently valid government-issued certificate (My Number card, driver's license, etc.) If you have a health insurance card (within the validity period) or qualification confirmation card, long-term care insurance card, etc. without a photo, two documents are required.)
