In accordance with the FY2018 revision of nursing care fees, from October 1, 2018, if a home service plan that includes home-visit nursing care centered on living assistance is created (changed) for more than a certain number of times as determined by the Minister of Health, Labor and Welfare, It is now mandatory to notify the city of the reason why home-visit nursing care (mainly lifestyle assistance) is required. If the number of visits per month exceeds the number specified by the Minister of Health, Labor and Welfare, please submit a notification form. Please note that verification may be conducted at regional care meetings, etc.

・Create Form 1 “Reason for using home-visit nursing care (mainly living support type) more than a certain number of times”.
・We will request the home-visit care business office to create Form 2, “About the current situation of users of home-visit care (life support-based) for a certain number of times or more.”
・Please submit Form 1 and Form 2 created above as well as the home service plan (Tables 1 to 4).
Note: If you provide home-visit nursing care (mainly lifestyle assistance) at multiple establishments, please request each home-visit nursing establishment to create Form 2.
If you are requested to create Form 2 by a home care support business, please complete it immediately and submit it to the home care support business.
Please submit by the end of the month following the creation (change) of the care plan.
Please submit it from the home care support office to the Nursing Care Insurance Section of the Elderly Welfare Division.
