Home care support establishments in which the utilization rate of the classification payment limit standard amount is 70% or more and home care services account for 60% or more of the services utilized.

1. The city requests the home care support office to submit the applicable care plan.
2. The home care support office will consider the appropriateness of using the care plan, write the reason why home care is necessary, etc. in the care plan, and submit it to the city.
3. The city will confirm the contents of the care plan and discuss the care plan at regional care meetings, etc., as necessary.
4. If it is determined during the discussion that a review is necessary, the home care support office will also reconsider the care plan in question and care plans with similar or similar content.
