Prior notification is required to install or change specified facilities based on the Noise Control Law and Vibration Control Law.
Specific facility
Specified facilities are facilities installed in factories, workplaces, etc. that generate significant noise and vibration and are specified by government ordinance.

If you are writing your own manuscript, please write with a ballpoint pen or fountain pen.
Please prepare two copies of the notification form: the original and a copy. You may use copy paper for copies.
Please refer to the examples for filling out and creating the notification form.
In the case of simultaneous notification of the Noise Control Act and the Vibration Control Act, notification drawings [4 to 7] of the Vibration Control Act can be omitted.
Note: Regarding various notification documents, if there is an error in the written contents due to the abolishment of seals, please correct them using double lines, etc.
Notes on submitting and returning notification forms
Please contact city officials in advance and have the person or person in charge planning to install the facility come to the city office.
In principle, a copy of the notification will be given to you after the completion inspection.
