●If you are writing your own manuscript, please write with a ballpoint pen or fountain pen. ●Please prepare two copies of the notification form: the original and a copy. You may use copy paper for copies. A copy will be given to you after the final inspection. ●Please refer to the examples for filling out and creating the notification form. Note: Regarding various notification documents, if there is an error in the written contents due to the abolishment of seals, please correct them using double lines, etc.
Notes on submitting and returning notification forms
●Please contact the city manager in advance and have the person or person in charge who intends to set up a designated workplace come to the city office. ●In principle, a copy of the notification form will be returned at a later date.
