When applying, we recommend that you create a LoGo Forms account using the link above.
By creating an account, parental information will be automatically reflected in the application form, eliminating the need to enter name, email address, etc.
After applying, you can check the details of your application from your My Page.

Usage registration period

If your application is completed by the 18th of each month, you can start using the service from the 1st of the following month.
Note : Applications cannot be accepted between midnight and noon on the 19th (or noon on the following business day if the 19th falls on a Saturday, Sunday or public holiday).
Note : The usage period is from the 1st of the month in which you start using the service to March 31, 2025. Even if you registered in the previous year, you must still register.

Usage fee and insurance premium

There is no charge for use. However, you will need to enroll in insurance for registration. The insurance premium is 624 yen (including annual amount and administrative agency fee).
Your premium will not change even if you register during the year. Additionally, premiums cannot be refunded even if you stop using the service during the year.
Note : Once registration is completed, refunds are not available, so please consider carefully before applying.

Other notes

• Please note that due to system maintenance on the site, the application process may be unavailable, so please apply well before the deadline.
• After registration is complete, a completion notification will be sent to the specified email address. Completion notification will be sent from no-reply@logoform.jp. Depending on the e-mail address you receive the e-mail from, it may end up in your spam box, so please make sure to set your e-mail settings in advance so that you can receive the e-mail.
• Please write the reception number listed in the registration completion email on the "first time use card" and bring it to the after-school children's classroom. An attendance card will be given to you on your first visit.

About snacks

Application period

As for snacks, you can use the number of snacks you have applied for from the 1st to the 20th for the next month.
Note : Once the snack application process has been completed, it cannot be canceled, so please discuss the number of snacks you would like with your child in advance.

Snacks are 110 yen per meal (including administrative fees).
Note : Please apply based on the number of meals you will use for the following month.

Payment for snacks will be made by credit card or PayPay.

Other notes

• Snacks will be provided on days marked with a circle in the snack column on the attendance card.
• On the snack application site, the contents and ingredients of the snacks to be provided the following month are posted. If your child has allergies, please be sure to check and do not order any items that contain allergies.
• It is not possible to apply by specifying one of the offered items. Also, you cannot specify which snacks to use.
• Please note that we will not provide snacks if there are more users than usual due to parents' meetings, etc. and we cannot secure a place to eat snacks.
