Regarding automobile fuel costs (gasoline, etc.) and taxi fare benefits for people with mental and physical disabilities

Inagi City Automobile Fuel and Taxi Subsidy Program for the Mentally and Physically Disabled (Children)
The city has a system in place for people with disabilities who have difficulty using public transportation such as trains and buses to receive benefits for automobile fuel or taxi fares to make daily life more convenient.
The following people are eligible for this system.
Target audience
Those who have one of the following grades in their notebook (1) Visual impairment grade 1 or 2 (2) Upper limb/lower limb or trunk dysfunction grade 1 to 3 (3) Internal disorder (heart, kidney, respiratory system, bladder, rectum)・Small intestine) 1st and 3rd grade (4) Internal disorders (immune/liver) 1st to 3rd grade (5) Hearing impairment 2nd grade (6) Love notebook 1st and 2nd grade (7) Cerebral palsy or progressive muscular atrophy
Grant details
On a monthly basis, you can choose between taxi, gasoline, and diesel fuel fare subsidies.
The maximum amount per period is 10,000 yen (200 liters if using gasoline).
Note: 1st period (April to July), 2nd period (August to November), 3rd period (December to March)
(1) Taxi fare is the amount used (only the fare is subsidized)
Receipts ( with dates written on them) of fares using the notebook-based fare discount system (displaying your notebook at the time of boarding will give you a 10% discount on fares) are eligible for the subsidy. Receipts that do not use the fare discount system are not eligible for the subsidy.
(2) Gasoline fee is 50 yen per liter (3) Diesel oil fee is 30 yen per liter Note: If you apply in the middle of each period or lose your eligibility, the amount will be 2,500 yen multiplied by the number of months in the period. This is the maximum payment amount.
Grant limit
The following cases are not eligible:
(1) If you are in a facility (2) If you are using it for commuting to work, school, or business
Usage report and billing
Payments will be made based on reports submitted three times a year (August, December, and April).
Please submit the status report along with receipts for fuel and taxi fares to the Disability Welfare Division by the deadlines listed below for each period.
1st period: April to July, must arrive by August 10th
2nd period: August to November, must arrive by December 10th 3rd period: December to March, must arrive by April 10th
Note: Deadlines for each period must be met.

Please prepare the following and apply to the Disability Welfare Division.
(1) Physical disability certificate or love notebook (2) Bank transfer address in the person's name
Regarding status reports etc.
From 2016, instead of mailing the annual status report from the city hall, please download and apply for the ``Situation Report'' and ``Receipt Attachment Sheets'' below.
We also distribute status reports at the Disability Welfare Division counter and Hirao/Wakabadai branch office. Please use this as well. You may also use copies of the forms you have.
Note: Hirao and Wakabadai branch offices will distribute and receive status reports, etc., but please contact the Disability Welfare Division directly for information on how to fill them out.
