コロナワクチン接種証明書(ワクチンパスポート)の発行

From April 1, 2020, it will no longer be possible to install vaccination certificate apps, issue vaccination certificates using vaccination certificate apps, or issue vaccination certificates at convenience stores.

For paper vaccination certificates, please continue to check the "About applying for corona vaccination certificate (paper)" below before applying.
In addition to the overseas certificates that have been accepted previously, you will now be able to apply for vaccination certificates for domestic use in Japan. For use within Japan, "vaccination certificate" and "vaccination record" can still be used as usual. No need to apply again.

Those who have been vaccinated against the new coronavirus using a vaccination ticket issued by Inagi City and need a certificate when traveling overseas.Please note that one certificate can be used both overseas and within Japan. Available.

Those who have received the COVID-19 vaccine using a vaccination ticket issued by Inagi City and require proof of vaccination
Note: For use within Japan, the "Vaccination Certificate" and "Vaccination Record" can be used as usual, so no application is required.

Issuance method

The certificate will be issued and mailed to you within about a week of receiving your application.

Application documents

Required documents

  • Application Form
  • Copy of passport (only for those seeking overseas certificates)
  • Copy of identification document with return address (driver's license, health insurance card, etc.)
  • A copy of something that shows the vaccination ticket number (vaccination ticket or pre-examination slip, etc.)
  • Copy of vaccination certificate or vaccination record
  • Return envelope (addressed and stamped)

Documents required depending on the case

  • Documents confirming your maiden name, alternate name, or alias (if your passport has your maiden name, alternate name, or alternate name, documents confirming your maiden name, alternate name, or alternate name)
  • If the request is made by a representative, a letter of attorney

Note: If someone other than the applicant applies, a power of attorney is required.
