Town name and lot number change certificate [Land]: This certifies the new and old lot numbers of the land.
Town name and lot number change certificate [Building]: This certifies the new and old house number and location.
(Certification is only possible in Hirao district (excluding Hirao Minami Ward) and Wakabadai district)
Certificate of change of town name and lot number [Corporate]: This certificate certifies the old and new location of the corporation.
Address change certificate: The Citizen's Division issues a certificate regarding the new or old address on your resident card. Have a look at this .

How to apply

An application form will be issued free of charge after filling out the required information at the Town Development and Revitalization Division counter on the 3rd floor of City Hall.
