Procedures for special permanent resident certificate issuance are accepted at the Citizens Section on the 1st floor of City Hall.
The alien registration certificate you have as of July 9, 2012 will be considered a "special permanent resident certificate" for the time being even after the new system comes into effect, and you can continue to use it. You will need to change your certificate to a special permanent resident certificate by the deadline.

If you lose your special permanent resident certificate (including a deemed special permanent resident certificate) due to loss, theft, destruction, or other reasons, please apply for reissuance within 14 days from the day you learned the fact.

You can apply for reissuance even if you wish to exchange your special permanent resident certificate, except in the case of damage. A handling fee of 1,600 yen will be charged, so please bring a revenue stamp (purchased at a post office, etc.) when issuing the certificate.
