Application for specific disease

If you suffer from any of the following diseases, we will issue a certificate of specified disease with a monthly copayment limit of 10,000 yen per medical institution.

1. Chronic renal failure requiring dialysis
2. Some congenital blood clotting factor disorders (hemophilia)
3. HIV infection resulting from administration of blood clotting factor products (due to blood products)
