<People who have been certified as needing nursing care (assistance) in Inagi City>
When completing the moving-out procedure, please return your insured certificate and burden-sharing certificate (or burden-limit certificate if you have one) to the Long-Term Care Insurance Section and Long-Term Care Certification Section of the Elderly Welfare Division and receive your benefit eligibility certificate .
Please submit the certificate of eligibility to the ward, city, town or village of your new address within 14 days of the date of the change of address. Your certification of need for nursing care (assistance) and your share of the burden will be carried over to the ward, city, town or village of your new address for six months, in principle.
<Those who have not been certified as needing long-term care (requiring support) in Inagi City>
When moving out, please return your insurance card to the Nursing Care Insurance Section of the Elderly Welfare Division.
<For those who have been certified as needing nursing care or assistance in the previous city, town, or village>
If you have been issued a certificate of eligibility from your previous city, town or village, please submit it to the Nursing Care Insurance Section and Nursing Care Certification Section of the Elderly Welfare Division when you complete the moving-in procedure.
If you have not been issued a certificate of eligibility for benefits from the ward, city, town or village where you moved, please state that you were certified when you completed the moving-in procedure.
If you submit or apply for a certificate of eligibility within 14 days of the change of address, your nursing care (assistance) certification and burden ratio will be carried over to Inagi City for six months, as a general rule.
<Those who have not been certified by the municipality before moving in>
After moving in, we will mail your insurance card to your home.
If you move into the next facility (a facility with special address exceptions) and change your address to that facility, the city, town, or village of your previous address will continue to be the insurer. However, community-based facilities (such as group homes) are not subject to the special address exceptions.
