Diaper Bag Supply Application Form

If you are not a user of the Second Cultural Center, the Fourth Cultural Center, or a facility user and would like to receive a diaper bag at the Third Nursery School or Fourth Nursery School, please fill out the application form in advance and contact the Environment Division. Please submit. Please note that if you do not apply in advance, you will not be able to hand it over.
