Get a Maternal and Child Health Handbook if you become pregnant

If you are diagnosed as pregnant, follow the instructions of the doctor who diagnosed you, submit a pregnancy notification form, and receive a "Mother and Child Health Handbook" and "Mother and Child Health Bag".
The Maternal and Child Health Handbook is useful for health management as it allows you to record the health status of mothers and children related to pregnancy, childbirth, and childcare. The "Mother and Child Health Bag" contains information necessary for childbirth and childcare, as well as a prenatal health checkup form.

Note 1: A power of attorney is required if a proxy other than a pregnant woman submits the notification.

Bring one of the following

  1. One document issued by a public office with a photo attached (personal number card, driver's license, passport, residence card, etc.)
  2. Two documents issued by a public office that do not have a photo attached (various health insurance cards, pension notebooks, basic pension number notifications, etc.)

Items required for pregnancy notification form

  • Pregnant woman's name, personal number, date of birth, address, telephone number, occupation
  • Current number of weeks pregnant, expected date of delivery
  • Presence or absence of health checkup, name and address of medical institution where pregnancy was diagnosed

Note 2: When coming to the counter, please check the reception hours of each place before coming.

Those who have an interview with a public health nurse will receive a childcare package (catalog gift worth 10,000 yen), pregnancy gift (electronic catalog gift worth 50,000 yen), etc.

At the Oyako Comprehensive Support Center, you can have an interview with a public health nurse at the same time you report (also available on Saturdays).
For more information, please see ``Childcare package'' and ``Pregnancy gift'' etc. will be given to pregnant women who have been interviewed .
