Notice of [1 year old gift]

Around the age of one, children's range of movement expands, and they become more assertive, which leads to more worries and consultations about childcare. The city will send out a questionnaire about childcare to families with children who have turned one year old, and will distribute 60,000 yen worth of child gift certificates (10,000 yen for those born before March 31, 2023) as a one-year-old gift to those who respond.
Please note that we may contact you after reviewing your responses to the survey.

Households with children who meet all of the following requirements

1. Children born between April 1, 2022 and March 31, 2025
2. Children who are registered as residents of Inagi City at the time of their 1st birthday
3. Children whose questionnaires arrive at the city by the day before they turn 2 years old and who are registered as residents of Inagi City at that time

From April 1, 2021 to March 31, 2025

Please fill out the “1-year-old child dental health checkup questionnaire” and submit it to the reception desk for the 1-year-old child dental health checkup. We will give you a 1 year old gift in exchange for receiving the questionnaire.
Note: If you do not have a 1-year-old child's dental health checkup, please return the "1-year-old child's dental health checkup questionnaire" in a self-addressed envelope. After receiving the questionnaire, it will be mailed to the address registered as a resident within two to three months .

Please fill out the "1-year-old child dental health checkup questionnaire" and return it in the enclosed self-addressed envelope. After receiving it, it will be mailed to the address registered as a resident within 2 to 3 months.
Note: The 1-year-old dental checkup is for the first child and multiple births.
