About mobility support business

What is the transportation support project?

We provide transportation support (dispatch of guide helpers) when going out for essential social activities, such as procedures at public institutions, and when going out to participate in society, such as leisure activities.

Target audience

Those who meet any of the following criteria: Holders of the "Love Notebook" or "Mental Health and Welfare Notebook" for the mentally disabled; School age or older

usage limit

This service is not available in the following cases:

(1) When going out for long periods throughout the year, such as commuting to work (however, this does not apply to children and adults who need to be accompanied to school and are unable to do so by their guardians).
(2) When going out for business or other economic activities.
(3) When going out for services provided under Article 8, Paragraph 2 of the Long-Term Care Insurance Act.
(4) When going out for medical treatment.
(5) Any other outing that is deemed socially inappropriate.

Utilization time

25 hours per month for those aged 18 or older; 15 hours per month for those under 18

User charges

User charges
User fee category \ Physical care can be none
Taxpayer 150 yen 90 yen
Non-taxable persons receiving welfare benefits 0 yen (exempt) 0 yen (exempt)

This is the self-payment amount per unit (30 minutes).

This is a list of Inagi City-designated businesses that have been used recently. If you would like to use a business that is not listed, please contact the Disability Welfare Division.

For business establishments considering new registration

About required documents

Please submit the following documents and use the service only after receiving the designation.
〇Mobility support service provider designation application form 〇Copy of one of the following proofs

  • care worker
  • Home care worker training course 1st or 2nd grade
  • Completion of nursing care worker training course (nursing care worker basic training course, visiting care worker (home helper) level 1 to 2 course)
  • Completion of training for care workers who provide transportation support for people with intellectual disabilities (only when providing transportation support for people with intellectual disabilities)
  • Deemed proof for each service

About Account Registration

If you do not have an account registered, please submit the following documents.

Billing

After designation, if you plan to use transportation support, please fill in the necessary information and submit it.

  • Invoice (business name is the same as the account registration)
  • Performance record sheet
  • Specification

About billing amount

With physical care: 1,500 yen per unit No personal care: 900 yen per unit If you receive personal care, it will be printed on your recipient card.
