Please download and use it as needed.
The request form can be submitted in any format as long as the items listed are complete.

Note: As of April 1, 2024, it is no longer necessary to affix a seal to payment account transfer request forms and invoices.

Note: Invoices in Excel format do not contain formulas for calculating amounts, etc.
Please enter the tax rate, calculation formula, etc. based on the contract as appropriate and use it.

Note: Although the invoice system was started on October 1, 2020, the city does not, in principle, require businesses to issue eligible invoices.
Note: However, for sewerage business accounting and hospital business accounting, qualified invoices must be issued by consumption taxable businesses that issue qualified invoices.
