2020 After School Children's Classroom Registration/Snack Application

You can register for use and apply for snacks using the application form.
If you have siblings, you must apply for each one separately.
You can use it 24 hours a day from your smartphone or computer during the application period.
Before using the service, please be sure to check the ``After-School Children's Classroom Handbook'' which contains details before registering.

When applying, we recommend that you create a LoGo Forms account using the link above.
By creating an account, parental information will be automatically reflected in the application form, so you can omit the need to enter your name, email address, etc.
After applying, you can check the details of your application from your My Page.

Registration period

If your application is completed by the 18th of each month, you can start using the service from the 1st of the following month.
Note : Applications cannot be accepted between midnight and noon on the 19th (or noon on the following business day if the 19th falls on a Saturday, Sunday, or public holiday).
Note : The usage period is from the 1st of the month in which you start using the service to March 31, 2025. Even if you registered in the previous year, you must still register.

Usage fees and insurance premiums

There is no charge for use. However, you will need to sign up for insurance when you register. The insurance fee is 624 yen (annual fee, including administrative fees).
The premium will not change even if you register for service in the middle of the fiscal year. In addition, the premium will not be refunded even if you stop using the service in the middle of the fiscal year.
Note : Once registration is completed, refunds are not possible, so please consider carefully before applying.

Other notes

• Please note that due to system maintenance on the site, the application process may be unavailable, so please apply well before the deadline.
• After completing your registration, you will receive a completion notification to the email address you specified. The completion notification will be sent from no-reply@logoform.jp. Depending on the email address you use to receive the notification, it may end up in your spam box, so please make sure to set it up so that you can receive it.
• Please write the reception number listed in the registration completion email on the "first time use card" and bring it to the after-school children's classroom. An attendance card will be given to you on your first visit.

Application period

As for snacks, you can use the number of snacks you have applied for from the 1st to the 20th for the next month.
Note : Once the snack application process has been completed, it cannot be canceled, so please discuss the number of snacks you would like with your child in advance.

Snacks are 110 yen per meal (including administrative fees).
Note : Please apply based on the number of meals you will use for the following month.

Other notes

• Snacks will be provided on days marked with a circle in the snack column on the attendance card.
• The contents and ingredients of the snacks to be provided next month are posted on the snack application site. If your child has an allergy, please be sure to check and do not order if there are any items that your child is allergic to.
• It is not possible to apply by specifying one of the offered items. Also, you cannot specify which snacks to use.
• Please note that if there are more users than usual due to parent-teacher meetings or other reasons and there is no space to eat snacks, they will not be provided.

If there are any changes to your registered details (address, school, etc.), or if you wish to leave mid-year due to reasons such as joining an after-school club, please notify each after-school children's classroom.
