If you are diagnosed with pregnancy, follow the instructions of the doctor who diagnosed you and submit a pregnancy notification form, and receive a Maternal and Child Health Handbook and a Maternal and Child Health Bag.
The Maternal and Child Health Handbook allows you to record the health status of mothers and children during pregnancy, childbirth, and childcare, and is useful for health management. The Mother and Child Health Bag contains the pregnancy health check-up slip as well as other information necessary for childbirth and childcare.

Note 1: If a proxy other than the pregnant woman is submitting the notification, a power of attorney is required.

Bring one of the following

1. One document issued by a public office with a photo attached (personal number card, driver's license, passport, residence card, etc.)
2. Two documents issued by a public office that do not have a photo attached (various health insurance cards, pension notebooks, basic pension number notifications, etc.)

• Name, personal number, date of birth, address, telephone number, and occupation of the pregnant woman
• Current pregnancy week and expected birth date
• Whether or not there was a medical checkup, and the name and location of the medical institution where the pregnancy was diagnosed
