[1 year old gift] Notice

Around the age of one, children's range of movement expands, and they become more assertive, which leads to more worries and consultations about childcare. The city will send out a questionnaire about childcare to families with children who have turned one year old, and will distribute 60,000 yen worth of child gift certificates (10,000 yen for those born before March 31, 2023) as a one-year-old gift to those who respond.
Please note that we may contact you after reviewing your responses to the survey.

Households with children who meet all of the following requirements

1. Children born between April 1, 2022 and March 31, 2025
2. Children who have reached their 1st birthday and are registered as residents in Inagi City at that time
3. Children whose questionnaire has arrived at the city by the day before they turn 2 years old and who are registered as residents in Inagi City at that time.

From April 1, 2021 to March 31, 2025

Please fill out the "1-Year-Old Dental Health Checkup Questionnaire" and submit it to the 1-year-old dental health checkup reception desk. We will give you a 1 year old gift in exchange for receiving the survey.
Note: If you do not wish to undergo a 1-year-old dental health checkup, please return the 1-year-old dental health checkup questionnaire in the return envelope. After receiving the questionnaire, it will be mailed to the address registered as a resident approximately within 2 to 3 months.

Please fill out the "1-Year-Old Dental Health Checkup Questionnaire" and return it in the enclosed return envelope. After receiving it, it will be mailed to the address registered as a resident within approximately 2 to 3 months.
Note: The 1-year-old dental health checkup is for the first child and children born with multiple births.
