If you wish to introduce the following services, you will need to consult with the city individually in advance.
(1) In-hospital assistance for home-visit nursing care (2) Living support such as home-visit nursing care services when there are family members living together (3) Regarding the lending of welfare equipment for people with mild symptoms (only if confirmation from the city is required)
(4) Use of short-term resident living care (short-term resident medical care) that exceeds half of the certification validity period (5) Other items that may require individual consultation Note: Please refer to the flowcharts and notices below regarding the necessity of consultation. Please check.

Note 1: Please take into consideration that it will take about two weeks for the review after receiving the submitted documents.
Note 2: Even if you are unable to submit your application before the deadline due to circumstances, please be sure to notify the Benefits Officer.
Note 3: If the service in question is determined to be "unsuitable for benefits" after its introduction, the service will not be eligible for nursing care benefits.
