You can request disclosure of your own personal information by submitting a retained (specific) personal information disclosure request form at the Information Disclosure Corner on the 5th floor of City Hall.

1. Please fill in the required items on the Request for Disclosure of Retained (Specified) Personal Information and submit it to the Documentation and Legal Affairs Section of the Document and Legal Affairs Division.
2. Please download and use the Possessed (Specific) Personal Information Disclosure Request Form from here ( Application Form Download: Information Disclosure, Personal Information Protection ) It is also available at the Documents and Legislation Section of the Documents and Legislation Division on the 5th floor of City Hall. In addition, when requesting disclosure of personal information including My Number, it is necessary to enter My Number.
3. When making a claim, it is necessary to present identification documents. In addition, when requesting disclosure of personal information including My Number, you will also be required to present documents confirming your My Number.
4. When making a request, it is necessary to specify as much as possible the administrative document containing your personal information. If you have any questions, please contact the Document and Legal Affairs Section of the Document and Legal Affairs Division in advance.
5. Please note that in order to protect the important information of the individual, we cannot make a request by phone or verbal.
6. There is no handling fee, but if you wish to have a copy or mailed, please pay the actual cost of the copy and postage. In addition, unless otherwise specified, we will use simple registered mail for mailing.
