If you wish to have the certificate issued by mail, please enclose the following four documents with your request.
(1) Postal request form (Please download from the application form download page "Family register certificate" )
(2) Handling fee (please pay with a fixed-amount postal money order at the post office) Note: Please inquire about the amount.
(3) Self-addressed envelope (write the address of the resident registration area and the name of the applicant, and affix a stamp)
(4) Identification documents, etc. (a copy of a driver's license, health insurance card, etc., as a document that can be used to confirm the applicant and the place of residence registration)
