If your name changes due to marriage, etc., you will need to complete the name change procedure at the Citizens Affairs Division. No procedures are required at the Insurance and Pension Division.
Note: For name changes for Category 2 and Category 3 insured persons, please notify the respective workplaces or spouse's workplaces.
You will need to submit a "Notification of Change of Pension Beneficiary Name."
The notification forms are available at the pension office and the city hall's insurance and pension department.
We will issue you a new pension certificate after your name change, so please attach all of your existing pension certificates and submit them.
You will also need to complete the name change procedure with the financial institution through which you receive your pension.
Note: If you are receiving a survivor's pension and the reason for the name change is marriage or adoption, you will no longer be entitled to receive the survivor's pension. Please contact the pension office for more information.
