Please go to the city hall or a branch office (Hirao or Wakabadai) to complete the enrollment procedure.

If you are not enrolled in company health insurance, etc., and fall under any of the following: (1) When you move from another municipality (2) When you quit your company (3) When you are no longer covered by social insurance (4) When a child is born (5) When the public assistance recipient is terminated

• Certificate of loss of social insurance eligibility (in the case of (2) and (3) above)
• Identity verification documents (driver's license, passport, My Number card, etc.)
