If the name on your My Number card for those who have emigrated overseas changes, you will need to change the name on the card.
You can complete the procedure at your local city, town or village.
Place of application (registered domicile is Inagi City)
- Inagi City Hall Citizens' Affairs Division, Hirao Branch Office, Wakabadai Branch Office (You can complete the procedure on the spot)
Note: If you apply at a Japanese diplomatic mission, you will be asked to apply for a reissue of your My Number card.

Application by the person
(1) My Number Card (You will need to enter your PIN at the counter)
Application by legal representative (if the person is under 15 years old or an adult ward) or spouse
(1) Your Individual Number Card (You will need to enter your PIN at the counter)
(2) One government-issued photo ID of the legal representative or spouse (such as a passport or My Number card)
(3) [If the person is an adult ward] Certificate of registered matters
important point
- If you are unable to enter a 4-digit PIN, you will need to reset your PIN. For details, please see "Changing and Resetting My Number Card PINs for People Emigrated Overseas."
- The electronic signature certificate installed on your Individual Number Card will become invalid if your name changes. If you wish to have a new one issued, you will need to come to the office in person to complete the procedure ( you will need to enter two PIN numbers: the 4-digit number and the electronic signature certificate number) .
