We are working to promote cooperation between medical and nursing care so that citizens can continue to live in the community until the end of their lives with peace of mind. Since 2015, the city has held the "Inagi City Multi-professional Cooperation Workshop to Promote Home Medical Care and Nursing Care Cooperation", and this year was the 5th time.
This year, under the theme of "Decision-making support supported by multiple occupations (ACP/life conference)", after a lecture on ACP by Ms. Miyuki Sato (home-visit nursing station Kesera) We shared our efforts and challenges for decision support. Many medical and nursing care workers participated in the group work, learning various perspectives and intervention methods from case studies and building relationships with face-to-face relationships. At the end of the training, Ms. Masako Akiyama (Hakujuji Home-Visit Nursing Station) gave a lecture on "comprehensive community care at home" and introduced initiatives for collaboration between home medical care and nursing care.
