The city is holding care plan review meetings as part of its efforts to optimize benefits.
This is done as part of the rationalization of benefits, but it is not a directive method, but rather uses a "reassessment support sheet" to start with an assessment (problem analysis) conducted jointly by the city, the chief care manager, and the care manager, encouraging the care manager to "realize" things, exploring together what "care management that contributes to independent living support" is, and aiming to universalize it and support the implementation of sound benefits. It also aims to improve the quality of care management by care managers.
Nursing care support specialists, home care support offices, small-scale multifunctional home care offices, nursing small-scale multifunctional home care offices belonging to the following offices located in the city:
(1) The city will request business establishments to attend the review meeting. (approximately 2 months before the scheduled implementation)
(2) The nursing care support specialist at the business will submit advance materials such as a care plan (pre-submission form, basic information sheet, re-assessment support sheet, home service plan) to the city. (Approximately 1 month before the scheduled implementation)
(3) We will hold a care plan review meeting and inspect the care plan. (Usually held on the afternoon of the third Tuesday of each month)
