Note : This does not apply to people who have ever undergone a hepatitis virus test.

Note: If you were born between April 2, 1980 and April 1, 1986, the city will send you a consultation ticket. There is no need to apply for the consultation ticket described below .

• The deadline is the 22nd of each month, and consultation tickets will be sent out at the beginning of the following month.

Note : The final application deadline is Friday, December 22, 2020.

Please apply for a consultation ticket at the Inagi City Health Center using one of the following methods (1), (2), or (3).

Please write the title ``Application for Hepatitis Virus Screening,'' fill out the following information, and mail it to ``Inagi City Health Center.''
