If you wish to change the location of your dog, the owner's address/name, owner, etc., or if your dog dies, please complete the procedures at the following counter.

Note: Procedures are possible from 8:30 a.m. to 5:00 p.m. on business days.
Please submit the "Notification of change in pet registration information".
Dog tag issued at the time of registration
・For procedures when moving to Inagi City, please bring the license issued by your previous municipality. If your ticket is lost, please be prepared to pay a 1,600 yen reissue fee.
・If you are moving outside Inagi City, you do not need to go through any formalities in Inagi City. Please bring your Inagi City license with you and apply to the local government you are transferring to.
Applications by mail will only be accepted from those who have the license issued by their previous municipality.
Please print out and fill out the form below, attach your pet's license tag, and send it to the Health Center. After a staff member checks it, we will send you the Inagi City license tag.
When a registered dog dies, please submit a ``Dog Death Notification''.
