Do I need to submit a birth notification card (postcard)?

Birth notification cards are necessary information to support infants and mothers.
Please notify us within 2 weeks after the baby is born using the birth notification slip.
You can also submit it using the two-dimensional code on the leaflet you received when you issued the Maternal and Child Health Handbook.
