If you have lost your ticket for a specific health checkup, please bring your health insurance card and photo identification (driver's license, my number card (personal number card), passport, basic resident registration card, or handicapped person's handbook) ) and come to the Insurance and Pension Division. Or you can call us and we can mail it to you.
