For the enrollment procedure, the identity verification document of the person who comes to the procedure is required.
If you do not have an identity verification document with a face photo, you may be able to complete the procedure even if you present multiple documents issued by a public office that do not have a face photo.
You can apply for the reissuance procedure even if you do not have your identity verification documents. In that case, the insurance card cannot be handed over at the counter, so it will be sent by simplified registered mail at a later date.
If someone from another household comes to complete the procedure, a power of attorney and identity verification documents for the person in charge are required.
